MP4 is a container, not a sound
An MP4 does not have one fixed audio format inside it. The container usually holds H.264 video alongside an AAC audio track, but it can carry several other combinations, which is why two files with the same extension sometimes behave differently in the same player. For transcription only the audio track is opened, so the video codec is irrelevant and none of the picture is ever decoded. This is also why an MP4 with no audio track produces nothing at all rather than an error that explains itself — worth checking first if a silent-looking file returns an empty transcript.
Where MP4s usually come from
The source shapes the result more than the format does. Phone recordings are the easiest case when the speaker is near the microphone and the hardest when they are across a room. Screen recordings are usually clean, because the audio was captured digitally rather than through air. Video-call exports vary a lot: platform-mixed recordings put everyone at similar volume, which helps, but they also stack overlapping speech into one track, which does not. Camera files from a real shoot are the best of all if a proper microphone was used, and among the worst if the on-camera mic was doing the work.
Screen recordings are their own job
A large share of MP4s people want as text are screen recordings — a demo, a walkthrough, a recorded standup, a lecture someone captured. The audio is typically clean, so accuracy is high, but the transcript alone is often not the deliverable. What people usually want is a written procedure: the steps in order, without the pauses and restarts of live narration. Read the transcript once against the recording and cut the verbal scaffolding, and a fifteen-minute walkthrough compresses into a page someone can follow without watching anything.
MP4, MOV and the rest
MOV is Apple's container and behaves almost identically for this purpose; anything recorded on an iPhone or exported from Final Cut will work the same way. MKV is more permissive about what it can hold, which is exactly why it occasionally carries an audio codec the browser cannot decode. WebM is the web-native option and always works. If any file refuses to load, exporting a standard MP4 from any editor fixes it, and since only the audio is used you lose nothing in the round trip.

If you only have the audio
